My GLP-1 cost log needs better columns than sticker price

I’m trying to make sense of the out-of-pocket side without turning it into a coupon-clipping hobby. My plan covers it, but the portal estimate swung from $150 to $287 between January and March, and the pharmacy receipt only labels it 'plan adjustment.' I started a little log: premium share, deductible remaining, 30- vs 90-day fill, whether the claim ran through medical or pharmacy benefit, and how many refills are left.

That last one surprised me — the per-fill cost changed once I crossed into the deductible, not because the medication changed. If you’ve got a system that actually predicts the bill, what columns or questions do you use?

I’m less interested in one-off hacks than in what reliably holds across a few months.

I added a “claim processed date” column next to “pickup date” after my January fill got weird. If the claim hits the new plan year before the deductible resets, or vice versa, the portal estimate goes sideways.

Also track which pharmacy location billed it — mine differed by $40 for the same script. And the running deductible/accumulator at the time of fill explained way more than the receipt’s “plan adjustment.” Does your portal show a running accumulator?

That’s the only column that made my log useful

I feel this—my receipt says “plan adjustment” and I still have no idea what changed. 😩

I’d add two columns: “quote source” (portal vs pharmacy counter) and “benefit accumulator after fill.” My portal estimate and counter price only lined up once I tracked accumulator progress—the swing was mostly deductible/plan-year timing, not the script itself. Also log whether the claim was reversed/re-billed; that’s different from the processed date and explains weird “plan adjustment” labels.

Sharp follow-up: was the $287 quote before or after your deductible-remaining updated? Portal lag makes it look random.

Not trying to coupon-clip, just reconcile the numbers.

I vote yes on a processed-date column—my receipt said “plan adjustment” too and I just sighed. 😅
