A $300-a-Year GLP-1 Membership Is Cheap. The Drug Is Where the Risk Lives.

Topics: GLP-1 telehealth, Counsel Health, LillyDirect, compounded semaglutide, tirzepatide, peptide testing, FDA warning letters, weight loss medication cost

A new telehealth membership puts physician access for weight management at roughly $25 a month — a fraction of what most digital clinics charge. But the membership is not the medication, and the medicine is where the cost and the safety questions actually sit.

What Happened

Counsel Health launched a weight-management program on Oct. 27, available directly to patients and through LillyDirect, Eli Lilly's direct-to-consumer pharmacy platform.

A $300 annual membership — about $25 a month — includes unlimited on-demand access to Counsel physicians for weight management plus general primary and urgent care. A $29 per-visit option exists without a membership. Medication costs are separate from both.

After an online intake and physician evaluation, Counsel clinicians can prescribe Zepbound, Wegovy, or Foundayo, Lilly's oral GLP-1 pill approved in April, when they judge it clinically appropriate. The company says patients reach a physician in about 15 minutes on average, with a supervised AI tool tracking progress between visits.

Who Is Affected

About 11% of U.S. adults say they currently take a GLP-1 drug for weight loss, up from 3% in 2024, according to Gallup polling. Many pay out of pocket and compare online providers like any major purchase.

Where they source their medicine matters more than the fee. In the same polling, 19% of current users said they take compounded or custom-mixed versions, and 12% were unsure which type they take.

Timeline

  • March 2026 — FDA sends 30 warning letters to telehealth companies over false or misleading claims about compounded GLP-1 products.
  • April 2026 — FDA approves Foundayo, the oral GLP-1 now offered through the program.
  • May 21, 2026 — FDA reports more than 1,700 adverse event reports tied to compounded semaglutide and tirzepatide.
  • June 2026 — FDA sends 25 more warning letters to telehealth firms on the same grounds.
  • Oct. 27, 2026 — Counsel Health opens its program directly and via LillyDirect.

What This Means for Researchers

There is no published outcome data on this program. Claims about response times, adherence, and side-effect monitoring come from the company, and both Counsel and Lilly have commercial interests in the arrangement. A fee that undercuts competitors on paper says nothing about whether patients stay on therapy.

Two structural points matter. The evaluation does not guarantee a prescription — a clinician may recommend lab work, a different drug, or no drug at all. And self-pay pricing remains the dominant cost: brand-name GLP-1 list prices still run into the hundreds per month without insurance, the gap that pushed many buyers toward unregulated suppliers.

How to Verify Your Peptides

If you buy from any online source, the medicine — not the platform — is what needs checking. Our testing guide covers what a legitimate certificate of analysis should show: third-party mass spectrometry for identity, sterility and endotoxin testing, and lot traceability. For compounded products, ask which licensed 503A or 503B pharmacy prepared the batch and request the lot number before paying. Compounded drugs are not FDA-approved; the agency has not reviewed their safety or quality.

Safe Alternatives

Brand-name products from a licensed pharmacy, with a prescription and a clinician monitoring tolerance, remain the benchmark pathway. Our peptide reference pages cover what is and is not supported by human trial data for compounds circulating in this market, and our vendor directory lists suppliers alongside the documentation they have provided. Neither replaces a prescriber.
